Appointments

General

All appointments can be arranged by calling 904-262-2249.

Our office hours are 8am to 6 pm and we are conveniently located just south of Loretto road on the right side of San Jose Boulevard.

Please arrive 5-10 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointments, please remember to bring the following

• Insurance Card
• Driver's License or Photo ID

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
